Host Home supports focus on helping individuals achieve personal goals, maintain their health and safety, and build skills needed for independent living. Supports may include assistance with daily living activities such as personal hygiene, meal preparation, household tasks, budgeting, transportation, recreation, shopping, and accessing community resources.

Beyond skill-building, Host Home Services emphasize meaningful relationships, social connection, and active participation in community life — creating a warm and stable setting where individuals can thrive.

Service Considerations

Host Home Services are designed as a primary residential support and may not be provided simultaneously with certain other residential or in-home services, except where specifically permitted. Service coordination ensures individuals receive the most appropriate level of care based on their needs and person-centered plan.
